No one writes a book on one’s own. This one, certainly, is the product of a myriad of discussions I’ve had with friends and colleagues over the years. As Isaac Newton once said, “If I have seen further it is by standing on the shoulders of Giants.” I want to thank my colleagues at the University of Western Australia for discussions that have helped to shape the arguments presented in this book, and indeed my thanks also go out to the University itself for providing me with a stimulating environment in which to write it.
